Limited to 1 gift card per household
Requirements
- MUST have experienced financial hardship due to COVID-19
- MUST be a North Miami resident, no exceptions. To verify your North Miami residency, please enter your address here
- MUST show proof of residency by providing a State issued ID containing a North Miami address matching the name and address on the application form
- Applicants will be considered ineligible if they provide an address outside of North Miami or an invalid form of ID
- Cards are issued on a first come, first serve basis while supplies last
- Only one application may be submitted per household

Forming vehicular lines are prohibited prior to 6:00 AM No Exemptions!
