U.S. Passport Application Requirements
| Adult Applicant (16 and older) |
- Proof of U.S. Citizenship
(One of the following):
- Valid Photo I.D.
(One of the following)
- Previously issued, undamaged U.S. passport
- Naturalization Certificate
- Valid U.S. Driver's License
- Valid U.S. Identification Card
- Current Government ID (city, state or federal)
- Current Military ID (military and dependents)
(Photocopies of front and back)
- Two passport-size photos (Photo service available in our office 2 passport photos for $10.00)
| Child Apllicant (16 and younger) |
- Proof of U.S. Citizenship
(One of the following):
- Evidence of Relationship (One of the following)
- Minor's certified U.S. birth certificate with both parents' names
- Minor's certified Foreign Birth Certificate with both parents' names
- Minor's Report of Birth Abroad with both parents' names
- Adoption Decree with adopting parents' names
- Court Order establishing custody
- Court Order establishing guardianship
(Photocopies of front and back)
- Valid Photo I.D. - Identification of Parents
(One of the following)
- Previously issued, undamaged U.S. passport
- Naturalization Certificate
- Valid Driver's License
- Current Government Employee ID (city, state or federal)
- Current Military ID (military and dependents)

When applying for a child's passport, under the age of 16 both parents must come in with the child if both are listed on the birth certificate.
